   Anthem, Inc. (ANTM) is a health insurance provider and an independent licensee of the Blue Cross and Blue Shield Association (BCBSA), a federation of 36 separate United States health insurance companies. It is headquartered in Indianapolis, Indiana, U.S. and it was originally formed as WellPoint, Inc. in 2004, when WellPoint Health Networks Inc. merged with Anthem, Inc. Ten years later, in December 2014., WellPoint, Inc. changed its name to Anthem, Inc. because this is the name that is most familiar to consumers.

   Anthem began as Mutual Hospital Insurance Inc. and Mutual Medical Insurance Inc., in 1946.

   In 1985, two firms known as Blue Cross of Indiana and Blue Shield of Indiana, merged into Associated Insurance Companies, In., later called ”The Associated Group”, a holding company. However, the usage of the name “Anthem” persisted.

   In 1995, The Associated Group set up Anthem Blue Cross and Blue Shield.

   In 1996, The Associated Group changed its name to Anthem Insurance Company.

WellPoint Health Network Inc. (formerly Blue Cross of California)

   Blue Cross of California, the predecessor of WellPoint Health Network Inc, was founded in 1982 with the consolidation of Blue Cross of Northern California (established in 1936) and Blue Cross of Southern California (established in 1937).

   In 1992, the Blue Cross of California creates WellPoint Health Networks to operate its managed care business.

   In 1993, the managed care business of the Blue Cross of California was spun off into a publicly-traded entity, WellPoint Health Networks Inc.

   In 1996, WellPoint Health Networks Inc. became the parent organization of the Blue Cross of California, which was restructured to a for-profit corporation.
